Agreement to Waivers

By checking (ticking) this box, I confirm that I have read, understood and agree to the three below waivers: Activity Waiver & Release; Medication Attention Waiver; and Infectious Diseases Waiver & Release.

ACTIVITY WAIVER & RELEASE

I understand that there may be some inherent risk of injury associated with my child’s participation in gymnastics and sports-related physical activities provided by The Little Gym® Europe and it’s franchisee The Little Gym® Godalming, Handy Cross and Windsor (together “The Little Gym® Europe”). I hereby waive any and all claims of liability and agree to release and hold harmless The Little Gym® Europe in the event that such an injury occurs to my child as a result of taking part in The Little Gym® Europe’s activities, unless and to the extent that such an injury or death is as a result of the negligence of The Little Gym® Europe or its employees, agents or representatives.

MEDICAL ATTENTION WAIVER

I hereby give my permission to The Little Gym® The Little Gym® Godalming, Handy Cross and Windsor to procure medical attention for my child in the event of an injury, accident, and/or illness to my child while participating in The Little Gym® Europe’s activities which, in the opinion of a paramedic or a medically trained practitioner, requires urgent medical attention and a parent, legal guardian or any given emergency contact are not available.

INFECTIOUS DISEASES WAIVER & RELEASE

I voluntarily enter the premises of The Little Gyms of The Little Gym® Godalming, Handy Cross and Windsor with my child and family members, and acknowledge that there may be inherent risk of exposure to, and contracting, infectious diseases, including but not limited to, influenza & COVID-19 associated with this and my child’s participation in The Little Gym® Europe activities. I have been encouraged by The Little Gym® Europe to practice good hygiene and appropriate measures including social distancing to reduce the likelihood of contracting or spreading infectious diseases. I have read, understand and agree to adhere to all of the rules, guidelines and procedures of The Little Gym® Europe COVID-19 protocol, which have been formulated to provide reasonable measures to avoid the transmission of infectious diseases. I have read, understand and agree to the full infectious diseases releases waiver of which this abbreviated waiver forms a part. I absolve and release The Little Gym® Europe and The Little Gyms of The Little Gym® Godalming, Handy Cross and Windsor from any liability with regard to the transmission of infectious diseases.
